Omnikey CardMan 4040 SmartCard Reader Driver

I've written the cmx(4) FreeBSD driver for the CardMan 4040 PCMCIA smartcard reader, using the vendor's Linux driver as a guide on how to talk to the hardware, discovering a userland triggerable Linux kernel buffer overflow in the process.

• Status: committed to current, MFC'ed to RELENG_7 before 7.1; ported to DragonFlyBSD.
